The GENERA ultrasound system is an innovative method of treating cutaneous ulcer lesions. Rapid radical debridement of the ulcer is obtained through the development of cavitation energy directly in contact with the lesion, removing even the most tenaciously adhered fibrin and necrotic tissue.
Due to the minimal transmission of mechanical effects and heat to the tissue the cavitation cleansing–removal procedure is painless if compared to the use of conventional debridement techniques.

 

The GENERA ultrasound system also achieves a lowering of the bacterial load of the infected ulcer; this effect is the consequence of the removal of adhering infected material and the direct antibacterial action on the bacterial membrane being subjected to permanent damage due to its implosion, provoked by the ultrasonic cavitation. Genera treatment does not interfere with other associated therapeutic measures.

The Genera system

This system comprises an ultrasound generator that operates continuously, capable of activating a titanium tip assembled on a piezoelectric transducer and vibrating at a frequency of 26 KHz.

 

With regard to the cleansing of a skin ulcer, there are five types of differently shaped tips available, each capable of producing a specific effect on the lesion to be treated.



 

Tips Codes 610 – 611 and 612 mainly generate an action aimed at detachment of the biological material (fibrinous-necrotic) adhering to the bottom of the ulcerative lesion and at reduction of the bacterial load of infected ulcers. Tip Code 654 performs an abrading action on the fibrinous material in the presence of irregularities of the bottom of the ulcerative lesion with absence of an actual detachment plane. The other 3 tips (Codes 613 – 682 – 666) allow cutting both tissues and small bone structures protruding from the bottom of the ulcerative lesion, which often have a negative effect on ulcer healing. The latter type of tip is used mainly to treat ulcerative lesions on the toes or related to forefoot amputations.

Each of the tips has the distal active part which, under ultrasound activation, is subjected to 26,000 excursions per second (26 KHz) with a transverse vibratory displacement of peak-to-peak amplitude between 15 – 30 µm. The active distal end of the tips is irrigated continuously with the aid of a peristaltic pump.

 

Application principles

The application principles of the Genera system in the treatment of ulcerative lesions depend on the development of mechanical-vibratory energy, cavitational energy and caloric energy. The mechanical-vibratory and cavitational energy generated by the tips allows detaching the fibrinous and necrotic material from the bottom of the ulcerative lesion, with its consequent removal and simultaneous bacterial killing effect. The use of caloric energy obtainable with ultrasonic activation of the tips further increases the system's efficacy in reducing the bacterial load in the presence of infected ulcers.

Owing to the minimal transmission of the mechanical and caloric effects to the tissues, cavitation makes the cleaning-removal procedure less painful than traditional debridement techniques.

The GENERA ultrasound system also produces a decline in bacterial load in infected ulcers. This result stems from the removal of the infected material adhering to the ulcer and from a direct antibacterial action caused by damage on the bacterial membrane, which undergoes implosion following the cavitational effect.

Treatment with Genera does not interfere with the other associated therapeutic measures.
